What To Bring With You

1. Picture identification for yourself and spouse, if married filing joint.	2. Social security card(s) or Taxpayer Identification number(s) for you, your spouse and all dependents.	3. Copy of last year’s tax return (very important – helps volunteer prepare this year ’s return).
4. Form W-2 (Wage and Tax Statement) from each employer.	5. Forms 1099-INT or 1099-DIV (to report interest and dividends).	6. SSA-1099 or RRB-1099 (to report social security and railroad retirement)
7. 1099R (to report pension income)	8. List of your medical, taxes, interest, contributions, and miscellaneous expenses (to itemize your deductions).	9. Child and disabled dependent care information.
10. For Elderly Rental Assistance Form – landlord information and amount of rent, utilities, and fuel paid in 2007.
